are also available through our mobile medical unit,

Specialty Clinic
The Specialty Clinic is located at the
South County Community Clinic
in Venice. It offers a range of
client-centered services linking eligible clients with health care,
psychosocial care, and other services in a timely and coordinated
manner. Services offered include Ryan White (Title II), AIDS Drug
Assistance Program (ADAP), case management, and others. Case management activities include:

Assessment of a client's needs and
personal support systems

Development of a service plan

Coordination of services to start plan

Monitoring and periodic reevaluation
of plan, including revision when necessary

HIV/AIDS
Education

HIV Prevention Education
SCHD offers HIV/AIDS prevention education
and risk reduction, provides condoms and educational materials, provides
technical assistance to community-based organizations, maintains a
resource directory of HIV/AIDS services in Sarasota County, works with
organizations to develop age-appropriate HIV/AIDS education course
curricula, and provides training through the
Office of Epidemiology and Disease Intervention Services.

HIV/AIDS Training Courses
HIV/AIDS training courses are
available for health care professionals, HIV testing and counseling
providers, and the community at large. These include courses required for various certifications,
like HIV-104, HIV-500, and HIV-501. Prior registration required.

Speakers Bureau
Trained SCHD speakers are available to
address local schools, community agencies, and other organizations on a
variety of HIV/AIDS topics. Please call (941) 861-2976 to arrange
a presentation.
People living with HIV/AIDS (PLWHA) are
also available to speak to local organizations. This program is
administered by CAN (Community AIDS Network). Please call CAN at

Surveillance
The purpose of the HIV/AIDS surveillance
program is to promote the ongoing, systematic collection, analysis,
evaluation, and dissemination of epidemiologic data. These data
are used to plan, implement, and evaluate HIV/AIDS programs and
interventions. Surveillance in Florida identifies HIV-related
conditions at various points along the spectrum of the disease from
reporting of HIV infection in otherwise asymptomatic people, to death
certificate review for HIV/AIDS-related mortality.
Florida state law requires health care
providers to report to the SCHD HIV/AIDS Surveillance Office any patient
diagnosed with any of the following conditions:

Western Blot positive or qualitative
PCR positive tests done after July 1, 1997.

Any CD4 tests with values below 200 or
14%
-
Any of the 26 AIDS-defining
opportunistic infections defined by the CDC
