Vital Records (Birth & Death Certificates)

Sarasota County Health Department provides certified copies of birth and death certificates to eligible individuals.  Stop by one of our locations or mail an application to the Ringling address.  A legal photo ID is required, along with the application. 

The cost for a birth certificate is $15.00 for the first certificate and $5.00 for each additional certificate purchased at the same time for the same individual.  

The cost for each death certificate is $10.00.  Death certificates are purchased in county where the death occurred. Sarasota County holds records from 2000-present. Note: If you require a death certificate for an individual who died prior to the year 2000, call 904-359-6900 x9000.
